For additional insurance providers:
Services may be covered in full or in part by your health insurance or employee benefit plan. I provide a receipt which you may submit to your insurance provider for reimbursement. You can check your out of network provider coverage carefully by asking the following questions:
- Important questions to ask your insurance company
- Do I have coverage for out patient mental health services?
- Do I have coverage for couples or family counseling and under what circumstances?
- How many sessions do I have per year and what date does it start over?
- Is there a deductible for mental health services separate from my health insurance deductible and how much is that?
- Do my mental health treatment costs apply to my medical insurance deductible?
- What is my co pay for mental health services?
- If my therapist is not a preferred provider, can I still receive coverage and how does it differ from that of a preferred provider?
- Is approval required by my primary physician?
Rates:
Initial Intake- $165 (1 ½ hours)
Return Visit- $115 (50 minutes)
*A reduced rate (Sliding Scale) is offered on a limited basis

Cancellation Policy:
If you do not show up for your scheduled therapy appointment, or cancel at least 24 hours in advance, you may be required to pay the full cost of the session.
